I. By indicating that I elect to participate in the use of Internet Fax Service (hereinafter Service) provided by FaxPipe (hereinafter Company), I apply for and subscribe to said service on a month-to-month or annual basis. 1) Monthly or annual service fee (based on the package I have selected).
2) One-time activation fee (non-refundable).
II. This monthly or annual service contract is automatically renewed upon receipt of payment or auto debit of authorized credit card. Payment is due by the first day of each monthly billing cycle, and said payment is considered late after five days from the initial due date of the billing cycle. The billing cycle date is the monthly anniversary date of initial account activation. A late fee of ten dollars will be assessed for payment after the fifth day from the first day of the new billing cycle. Service will be temporarily discontinued if payment has not been received by the fifteenth day of the service month. Reactivation of accounts requires an additional ten dollar fee for administrative data processing. Payment will be made using authorized credit cards kept on file in the account, or with mailed-in Cashier's Check or Money Order.
III. I acknowledge that there are only two (2) methods of payment for the service fees and the overage page costs, and that all financial transactions shall be made in US Dollars:

Unless otherwise specified in a particular account offering, these included pages can be used in any combination of inbound and outbound faxing, totaling the chosen allotment. Overage fees are charged for pages sent/received in excess of the monthly allotment of pages included in the package selected. Overages are charged 15 cents per page on annual accounts (25 and 75 pages per month) and 10 cents per page on monthly accounts (300, 600 and 900 pages per month).
